Requirements:
Must be licensed as an Occupational Therapist in Arizona
Must possess a Valid Driver's License
Must possess or be able to obtain Level 1 Arizona Fingerprint Clearance Card
Must have reliable transportation
Must be willing to travel to member's homes or community locations for ongoing treatment
Must be open to teletherapy options based on members preference

Duties:
Evaluate individuals for occupational therapy services
Develop a Plan of Care based on the members needs and goals allowing them to reach their full potential
Review and Update Plan of Care Annually
Provide ongoing treatment for individuals as outlined in Plan of Care
Document SOAP notes electronically
Complete Client Progress Reports electronically
Effectively communicate with clients, families, supervisors and COTA's, etc.
Excellent written and verbal communication skills
Must be a Self-Starter
Spanish speaker is a strong plus
