Direct Support Professional/Caregiver
Acacia Care LLC, a HCBS agency based in Phoenix, AZ is expanding its team. We provide high-quality services to individuals with developmental disabilities.
As a rapidly growing company, we are currently seeking Direct Support Professionals to provide services to our members with developmental disabilities in both home and community settings.

As a Direct Support Professional, you will make a difference in someone’s life every day. DSPs provide support and strategies to the individuals we serve to live as independently as possible. DSP’s provide Respite, Habilitation and/or Attendant Care Services.

Respite Provider:
• Duties include providing the family/immediate caregivers a break. This service is solely intended to supervise, care for, and to entertain the member to ensure the parents/guardians are receiving reprieve.

Attendant Care Provider:
• Duties include providing total care to members for basic daily living needs such as: transferring from bed to wheelchair, bathing, clothing, feeding, and more.

Habilitation Provider:
• This service is intended to enable individuals to acquire knowledge and skills, increase or maintain self-sufficiency, provide training/assistance in essential activities required to meet personal and physical needs, and maintain the health and safety of the individual.

Qualifications:
• Must be at least 18 years of age
• High School Diploma or GED equivalent
• 3 Written References
• Clear Criminal Record
• 3 months of experience in human services
• Smart Phone for emergency contact as well as clocking in/out
• Email Capability
• Reliable Transportation
