**Excellent Career Opportunity with Banner Health in the Greater Phoenix Area!** Banner Health, a Top 5 Large Health System and one of the nation's leading non-profit healthcare networks, is expanding its team of Advanced Practice Providers in the rapidly growing city of Surprise, AZ. **We are seeking an experienced Family Nurse Practitioner (FNP) to join our primary care team, focused on delivering value-based care through a virtual telehealth platform** . We are looking for someone comfortable with telehealth driven care that will also support fellow clinical team members thru high quality, team based virtual care.

**Some Details About the Position:**

+ Clinic based onsite in Surprise, AZ

+ Flexible scheduling: 4x10s or 5x8s shifts

+ Medical Assistants,Front Office, and leadership support

+ Support of an integrated system including specialists, urgent care, and more!

**Essential Functions and Requirements:**

+ Provide comprehensive, high-quality care and support to established patients via tele visits

+ A portion of time will be of support of inbox management for your team members

+ DEAwith prescriptive authority required

+ FNP designation with AZ license eligibility

+ Minimum (1) year of clinical experience as an FNP in a primary care setting

+ Patient-focused, friendly, and willing to collaborate in a team environment
