Children's Case Manager - West Valley Behavioral Health Center:
JFCS is seeking enthusiastic and empathetic Case Managers to assist children who may be receiving behavioral-health services on the path to a more self-sufficient lifestyle and/or family reunification. The Case Manager will also provide guidance and support to the families/caregivers of these youth as needed.
Duties include:
Provides Case Management services to assigned clients.
Follows required guidelines for monthly contact, quarterly home/community visits, attending medication checks, etc.
Assists clients in finding appropriate services and works with children to decrease behaviors and improve stability in the community & home.
Responsible for coordinating and facilitating Child & Family Team (CFT) meetings.
Collaborates with clinical team to ensure that Integrated Service Plans, CFT meetings, and all required documentation are completed accurately and in a timely manner.
Collaborates and coordinates services with other involved agencies, such as the Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), schools, and other providers.
Completes referrals, prior authorizations, and other paperwork as required to secure appropriate services for clients.
Will inquire about the client’s health & wellness needs to assist in coordinating primary care or other supportive services with appropriate JFCS staff or external parties to ensure the client’s whole health is addressed.
Qualifications:
-Must meet one of the following education/experience requirements:
High School Diploma/GED required and 4 years of Behavioral Health experience
A.A. Degree and two years of Behavioral Health experience
Bachelor’s Degree in Social Work, Counseling, Marriage and Family Therapy, Psychology or related field.

Level 1 Fingerprint Clearance Card without Restriction(s)
Meet Agency requirements for MVR (Motor Vehicle Record) report
