Responsibilities Job Summary The Pharmacy Medication Reconciliation Technician will improve medication safety by assisting pharmacists in collection and documentation of up-to-date medication lists for patients being admitted to the organizations hospitals in order to optimize the medication reconciliation process and reduce the number of medication errors secondary to inaccurate or incomplete information. Responsible for obtainingand verifying medication history in the electronic medical record in an accurate and timely manner in compliance with standard procedures and regulatory standards. Responsible for accurately documenting medication lists into the electronic health record, communicating to the verifying pharmacist, and relaying any outstanding transition of care issues to the pharmacist and/or provider in a timely manner. Performs other duties as assigned. Qualifications Education High School Diploma or GED Required Experience <10 months pharmacy technician experience or completion of a formalized technician training program. Required Licenses and Certifications Certified Pharmacy Technician (CPT) Current license by AZ State Board of Pharmacy. Required Pharmacy Technician Certification Board (PTCB) Current Pharmacy Technician Certification (CPhT) issued by Pharmacy Technician Certification Board (PTCB). Required
