Axiom Care is a fast-paced, rapidly growing substance abuse treatment and sober living housing provider with locations in the Phoenix metro area and Apache Junction. We primarily serve low-income and justice-involved individuals who are seeking to change their life for the better. We achieve this by offering multiple levels of care along with medical services, evidence-based professional counseling, case management, housing assistance, employment assistance, and more.
Position Overview
While providing group therapy, participates in interdisciplinary treatment planning, and will provide counseling through development of discharge planning and other Case Management activities while supporting the therapeutic assessments and modes of therapy utilized by behavioral health professionals to assist client with substance abuse issues.
Counselor I’s work closely with case managers, clinicians, other BHTs, and clinical administration in the clinical department to provide high quality services to the OP population at Axiom Care. Those in this role provide direct services including psychoeducational group counseling, psychoeducational individual counseling, other therapeutic interventions in their scope, conduct biopsychosocial assessments, crisis intervention and discharge planning as needed. Indirect service includes meeting with their care team, attending oversight and training, completing other documentation as needed. Counselor I’s complete professional clinical documentation in the EMR and work with the billing department for all necessary utilization review tasks.
Duties/Responsibilities
Maintain strict adherence to state and local ethical standards and legal requirements at all times.
Provide high quality, compassionate, and ethical substance abuse treatment to clients in group and individual settings.
Develop client-driven individualized treatment plans and continually monitor treatment progress.
Complete high-quality documentation of any service provided or other clinically pertinent information within 24 hours.
Maintain good standing with the appropriate board, licensure, and insurance entities, if applicable
Utilize CBT, motivational interviewing, positive psychology, trauma informed care and/or other evidenced based techniques within scope to help clients achieve their treatment goals.
Take initiative to be aware and updated about community resources.
Assess client progress, needs and barriers in each interaction and document.
Make referrals to and assist in coordinating care with external sources when indicated.
Secure client information per HIPAA standards.
Assist with providing and attends training as requested.
Attend clinical meetings, oversight and interdisciplinary team meetings as requested.
Collaborate with the interdisciplinary team throughout the day including tech staff and leadership to ensure the best client care and open, frequent communication.
Assist with case management functions as needed.
Assist in holding clients accountable for attendance and compliance with program expectations.
Identify and report ongoing maintenance and safety issues at facility.
Intervene with de-escalation, empathy and, as needed, emergency response protocol to crises and emergency situations.
Perform other related tasks and projects as assigned by management.
Required Skills/Abilities:
Proficient in computer applications and electronic medical record systems.
Proficient with Microsoft Office Suite and related software.

Qualification Requirements
Minimum of one year of clinical experience with the AHCCCS insured population, justice involved adults, chronically underhoused individuals and/or adult substance use treatment.
Experience with group facilitation.
Bachelor’s Degree in Psychology, Social Work, Marriage and Family Counseling, Counseling or relative field, from an accredited college or university, or equivalent work experience strongly preferred.
Two to three years of experience working in behavioral health or substance abuse field preferred.
Strong computer skills required, experience with electronic medical records preferred.
Ability to communicate effectively with clients, their support system, and other professionals.
