Responsibilities Job Summary The Oncology Nurse Navigator (ONN) is a Registered Nurse with oncology-specific clinical skills and comprehensive knowledge of cancer, treatment modalities, and cancer management. The ONN will orient and educates patients, families, and caregivers to the healthcare system, multidisciplinary team member roles and resources. The ONN utilizes advanced clinical thinking and analytical skills that contribute to patient adherence of treatment protocols and symptom management. The ONN will demonstrate knowledge of NCCN clinical guidelines and specialty resources to assist patients making complex clinical decisions regarding the best recommended practice and ensure that appropriate standards of care are incorporated into the treatment plan. The ONN will serve as the central point of contact to the patient facilitating collaboration among all team members and ensuring individualized approach from diagnosis, active treatment through survivorship. The ONN will demonstrate commitment to quality patient care, implement creative and innovative ways to meet diverse patient needs and ensure best practices. Able to develop collaborative relationships both internally and externally. Contributes to the development of nursing research using evidence based research to identify strategies for improving professional development activities, nursing practice, and patient outcomes. Possess a comprehensive knowledge base of pathophysiology of cancer, treatment modalities, disease progression, and system management which leads to improved patient outcomes. Assess patient needs upon initial encounter using appropriate tools (ie: distress thermometer, pain scale) and identification of appropriate patients for genetic counseling. Navigates patient to appropriate services, referrals to health team and support services from active treatment into survivorship. Utilizes advanced clinical thinking and analytical skills to assemble and critically examine the evidence necessary for complex, autonomous clinical decision making that contributes to improved outcomes for patients and families. Provides education and resources for patients to facilitate informed decision making and timely access to quality health and psychosocial care throughout all the phases of the cancer continuum. Demonstrates knowledge of clinical guidelines (NCCN, AJCC) through all phases of patients care plan and survivorship. Able to implement survivorship care plans with patients who continue to receive treatment to reduce the risk of recurrence or to manage the cancer as a chronic disease. Navigator will empower survivors to establish maintenance and life goals striving to diminish the negative impact of cancer care physically and emotionally. Offers individualized assistance to patients, families and caregivers to overcome barriers that delay access to care and the healthcare system using the nursing process. Collaborate and build partnership with local agencies, healthcare facilities, community and organizations to improve and assist with patient care, support and education Measure and evaluate trends in patient responses and needs, retention, along with micro and macro level changes affecting specific patient populations Qualifications Experience 5 years Nursing experience Required 3 years Oncology experience Required Licenses and Certifications Registered Nurse (RN) State And/Or Compact State Licensure Required
