The Treatment Coordinator provides ongoing case coordination services that foster solution/strengths-based orientation to assist in addressing the client and family’s ongoing service needs for both primary care and behavioral health.

Duties include:

*Responds to initial crisis calls and identifies needs & necessary resources accordingly.
*Uses established best practices to engage formal and informal resources for the client, including the transition, discharge, and aftercare plans of behavioral-health services and demonstrates a positive, strengths-based attitude of teamwork and shared leadership.
*Performs ongoing case management and coordination of services to children and adults within the Child & Family Team process in accordance with the Plan.
*Assists clients and families in obtaining community support, identifying resources, and resolving barriers.
*Provides coordination of behavioral-health services with other agencies, such as Department of Child Safety, Probation and Parole Departments, Division of Developmental Disabilities, Arizona Long Term Care, schools, and other providers.

Qualifications:

*Bachelor's Degree in behavioral health field preferred; a combination of experience and education may be considered
*Relevant experience working in a behavioral-health setting
*Level 1 Fingerprint Clearance Card
*Verification of education, employment, references
*MVR matching JFCS driving guidelines
*AZ Driver's License and Valid Auto Insurance Reliable
