Summary

Outreach Engagement Worker is responsible for engaging individuals who have been identified by hospitals, emergency personnel, families, faith community and other social service agencies as having a substance use disorder, specifically those with Opioid Use Disorder (OUD), into treatment services. These services are provided as part of MATs 24/7 programming. Work hours may include shifts outside of regular hours on weekends, evenings or overnights to meet program needs.

Essential Duties and Responsibilities

Perform outreach at homeless camps, shelters, emergency departments, detox centers, and other locations, with priority given to pregnant and parenting women, IV drug users, homeless individuals and those involved with the criminal justice system. These services are provided pre-enrollment to MAT.
Build and maintain professional relationships with local service organizations, hospitals, police, etc.
Provide individuals with appropriate referrals to community programs and services.
Provide follow up on all referrals to assess outcomes and additional services as required.
Conduct initial screening of individuals for eligibility for MAAT services and makes appropriate referrals.
Coordinates intake and enrollment with the appropriate CODAC staff by supporting individual until enrollment is completed.
Track and maintain written records of community contacts and outreach for potential enrollments.
Inform and educate members regarding the meaning and implications of test results and prescribed interventions, as per clinic protocol.
Consults and cooperates with community systems to facilitate linkage, referral, crisis management, advocacy, and follow up with the focus on attaining treatment goals.
Provide support and/or assistance to other departments to ensure service engagement for MAAT.
Performs other related duties in accordance with agency growth and changes
SKILLS / REQUIREMENTS
Education:

Masters or Bachelor’s degree in a healthcare field and no related experience OR;
Masters or Bachelor’s degree in a non-healthcare related field and 1 year relevant related experience OR;
Associates’ degree in a healthcare field and 1 year related experience OR;
Associate’s degree in a non-healthcare field and 2 years related experience OR;
Certified Peer Support Specialist and 2 years related experience OR;
High School diploma or General Education Equivalent and 4 years related experience OR;
Is an Arizona Licensed Registered Nurse OR;
Is an Arizona Licensed Physician’s Assistant who is not working as a medical practitioner;
Experience:

Preferred Qualifications:

Two (2) years working with Medication Assisted Treatment (MAT).
One (1) year of experience working with individuals with substance use disorders.
Emergency Medical Technician (EMT)
Certifications:

Commission Certified Care Manager (CCCM), a plus.
Certified Family Support Partner, a plus
Certified Peer Support Specialist, a plus
Additional Requirements:

Valid Arizona Driver’s license, proof of current insurance and willingness to use personal vehicle. Driver’s license is an essential requirement for performing duties for this position.
Clean Motor Vehicle Record - no more than 2 moving violations or a license suspension in past 3 years.
