Responsibilities

Job SummaryThe ASSOCIATE COUNSELOR- BEHAVIORAL HEALTH (NOAH) is responsible for providing outpatient counseling services to clients and their families. Under clinical supervision provided by the organization, responsible for intakes, assessment, treatment and discharge planning, individual, family and group counseling, and family support services. Charts progress, maintains clinical records, and ensures implementation of treatment and discharge plans. Monitors and follows up on family needs, does re-assessments as required or as needed, and submits weekly and monthly service reports. Coordinates services as necessary working in collaboration with various State, Federal and other service partners included in NOAH. Services are provided according to all licensure, state and federal laws, and NOAH policies and procedures.

+ Provides family centered outpatient clinical services to families. Provides intervention in the office, or other locations as determined by treatment plan. Performs client admissions for new and/or returning clients. Develops an assessment of the family utilizing input from family as well as observation. Works with the client and family to develop and implement a treatment plan. Provides clinical services including individual, family and group counseling. Able to effectively and insightfully address a variety of complex issues related to individual, families, and socio-cultural variables. Utilizes professional resources as appropriate (e.g., case conferences, consultants, ongoing supervision and relevant literature). Tracks progress of treatment plan and updates as needed. Advocates on behalf of families as needed. Stays abreast of, and utilizes community resources. Initiates discharging planning as necessary.

+ Participates in a team management approach to patient care/treatment; work with NOAH team to coordinate services that meet the goals and objectives of the NOAH program. Maintains appropriate clinical documentation in a timely manner. Maintains quality care through participation in peer review, policy development and quality improvement programs.

+ Participates in continuing education to maintain professional standards. Participates in Performance improvement activities, supports membership in professional organizations.

Qualifications

EducationMaster's Degree in social work (MSW), counseling (MC) or other behavioral health related field RequiredLicenses and Certifications Must be Licensed Associate Counselor (LAC), or Licensed Masters Social Worker (LMSW) and in good standing with the Arizona Behavioral Board of Health examiner's, in the process of accruing hours with a goal to obtain independent licensure Required

Fingerprint Clearance Card - State current Fingerprint Clearance card or within seven working days after employment shall apply and be eligible for a Fingerprint Clearance Card Required
