The Nurse Practitioner (VSP/PT) provides health promotion and maintenance through the diagnosis and treatment of acute illness and chronic conditions.

**Responsibilities**

**Patient Care** :

+ Evaluates and treats center patients in accordance with standards of care.

+ Follows level of medical care and quality for patients and monitors care using available data and chart reviews.

+ Assists in the coordination of patient services, including but not limited to specialty referrals, hospital and SNF coordination, durable medical equipment and home health care.

+ Acts as an active participant and lead with the care team through daily huddles.

+ Helps Regional Medical Director (RMD), Physician and Center Administrator in setting a tone of cooperation in practice by displaying a professional and approachable demeanor.

+ Completes all medical record documentation in a timely manner working with a quality- based coder to optimize coding specificity.

+ Follows policy and protocol defined by Clinical Leadership.

+ Meets with RMD about quality of care, review of outcome data, policy, procedure and records issues.

+ Participates in potential growth opportunities for new or existing services within the Center.

+ Participates in the local primary care “on-call” program of CenterWell Senior Primary Care.

+ Assures personal compliance with licensing, certification, and accrediting bodies.

**Required Qualifications:**

+ Minimum of two to five years directly applicable experience preferred

+ Successful completion of a Nurse Practitioner program with Board Certification

+ Active, unrestricted Nurse Practitioner license in state of practice location

+ Excellent communication skills; written and verbal, demonstrate a high level of skill with interpersonal relationships and communications with all colleagues; fully engaged in the concept of “Integrated team based care”.

+ Willingness and ability to learn/adapt to practice in a value based care setting.

+ Superior patient/customer service.

+ Basic computer skills, including email and EMR.

+ Appropriate certification to write prescriptions under the authority of a collaborating physician/medical director

+ This role is considered patient facing and is a part of our Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

+ Active and unrestricted DEA license

+ Medicare Provider Number/ Medicaid Provider Number

+ Experience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value based relationship environment.

+ Knowledge of Medicare guidelines and coverage.

+ Knowledge of HEDIS quality indicators.

+ Bilingual is a plus.

**Responsibilities**

A Physician Assistant is a crucial member of the healthcare team who works under the supervision of a licensed physician. Their role is multifaceted and involves a range of clinical and administrative responsibilities aimed at providing high-quality patient care. hysician Assistant Associates work in various healthcare settings, including hospitals, clinics, primary care practices, specialty offices, and urgent care centers. + Provides diagnostic and therapeutic services to patients under supervision and responsibility of a Physician.

+ Gathers complete history of patients and perform complete physical examinations or focused physical examinations for specific complaints. Completes discharge summaries and maintains documentation on patient visits

+ Conducts daily patient rounds, as needed

+ Recognizes and evaluates situations which require immediate attention of supervising physician and which require treatment procedures essential for life of patients; responds in appropriate manner.

+ Coordinates with supervising physician regarding patients seen such as sees patients in physician's office and hospital, instructs and counsels patients on health and disease issues based on assessment data

+ Follows up with patient by phone (post treatment). Coordinates resources to facilitate care for out-of-town patients

+ Prescribes and dispenses drugs and medical devices in accordance with regulations

+ Monitors effectiveness of therapeutic interventions and makes appropriate referrals when necessary.

**Responsibilities**

**_Nurse Practitioners and Physician Assistants in the Department of Urology on the Phoenix campus work within care teams comprised of both physicians and NP/PAs working collaboratively._** In a care team model, NP/PAs are partnered with individual physicians to support patient care needs of the team. In addition to supporting team based care, the NP/PA also maintains their own clinic with individual patients. Together, the teams provide care for a wide range of patients across subspecialties including genitourinary oncology, female pelvic medicine and reconstructive urology, medical urology, endourology, and men’s health and infertility.

**The NP/PA role in Urology requires outpatient clinic only.** There are no call or hospital rounding activities or assignments. NP/PA staff are in clinic daily with no weekend responsibilities. Within the clinical setting, NP/PAs provide evaluations and consults with indirect supervision and may be involved in outpatient procedures. Additionally, the NP/PA team interacts and partners with resident and fellow staff, and is actively engaged in educational activities with rotating NP/PA students and the department's developing Urology NP/PA Fellowship Program.

**Responsibilities**

The Physician Assistant provides clinical services in a hospital setting and work as a first assist in the operating room or perform complex procedures on a regular basis (i.e., daily/weekly), which is greater than 50% of their time. Complex procedures are considered to have a major impact on hemodynamics, fluid shifts, or possible major blood loss requiring a high acuity setting. This position will be required to participate in the on-call rotation and provide coverage for various shifts, weekends, and holidays.

Physician Assistants provide primary and specialty health care to new and established patients, assess and manage common acute and chronic illnesses, consult and make referrals, and collaborate with physicians and other members of the health care team. The provision of care includes, but is not limited to obtaining patient history, comprehensive assessment, ordering, conducting, and interpreting diagnostic tests, and prescribing pharmacological agents and treatments and non-pharmacological therapies. Monitors the effectiveness of the therapeutic interventions. Makes appropriate referrals. Performs minor procedures as approved by the AZ Regulatory Board of Physician Assistants. This individual must demonstrate knowledge of the principles of growth and development of the life span. Provides excellent care under the direct supervision of staff physicians in the role of Staff N.P./P.A. for the patients of Dignity Health Urgent Care Clinics.

• Develops, implements, and maintains recordkeeping systems and procedures; gathers data, compiles information, and prepares reports for SAMHSA.
• Applies subject matter expertise across the department to ensure program success and ensure compliance.
• Direct the work efforts of individual program contributors, student employees, or temporary workers.

Administrative support and operations - 30%

• Makes decisions in partnership with Key Personnel related to allocating grant resources on a day-to-day basis.
